Doc Talk #87 – The Medical Home

 

 

In this show, Dr Dan and Jamieson discussed the concept of the health / medical home. This is a model of care advocated as the future of family practice in Canada and elsewhere. It is a team-based approach to care, highlighting access, continuity of care and multidisciplinary care to enable patients to enjoy higher quality healthcare and better outcomes.

We talked about the case of Michael and Michaela, their interactions with the healthcare system and how Michael benefited from having a Medical Home.
